About this template
1. Prepare for the Review a. Gather Relevant Information - Performance Data: Collect quantitative and qualitative data, including work outputs, project outcomes, and any relevant metrics. - Previous Reviews: Review past performance evaluations to assess progress. - Feedback: Compile feedback from colleagues, clients, and other stakeholders. - Self-Assessment: The employee should complete a self-assessment prior to the review. b. Review Company Standards and Objectives - Ensure alignment with organizational goals, values, and performance standards. 2. Completing the Template a. Fill in Employee Information - Accurately enter all basic details to identify the review context. b. Evaluate Performance Criteria - Assign Ratings: Objectively rate each performance area using the provided scale. - Provide Comments: Include specific examples and situations that illustrate performance levels. - Positive Feedback: Highlight strengths and accomplishments. - Constructive Feedback: Address areas needing improvement with clarity and sensitivity. c. Review Past Goals - Assess Achievement: Determine if previous goals were met, partially met, or unmet. - Discuss Obstacles: Identify any challenges faced and how they were addressed. d. Set New Goals and Objectives - SMART Goals: Ensure goals are Specific, Measurable, Achievable, Relevant, and Time-bound. - Collaborative Planning: Involve the employee in setting realistic and motivating objectives. e. Develop a Development Plan - Identify Opportunities: Suggest training programs, workshops, or mentorship opportunities. - Resource Allocation: Determine what support and resources are needed. - Timeline: Establish clear timelines for development activities.
